FACTS Survey

HOME > FACTS SURVEY

FACTS is the Flagstaff community’s before- and after-school program for children ages 5 to 12. Providing a safe and enriching environment for children benefits our entire community. We invite you to complete this brief survey to help us improve our services and meet the needs of your business and your employees. The survey will take less than three minutes to complete. We’d sincerely appreciate your input.

FACTS is supported by the Alliance for the Second Century (City of Flagstaff, Coconino Community College, Coconino County, Flagstaff Unified School District and Northern Arizona University)

FACTS serves an average of 670 children every school day.

FACTS is open to all kindergarten through sixth grade children in the community, regardless of the school they attend (district, private, charter or home-schooled).

FACTS operates in the 12 elementary schools in the Flagstaff Unified School District.

FACTS provides up to 25 hours of direct student services every week in the hours before and after-school.

FACTS is open from 6:45 a.m. to 6:00 p.m. on many school holidays, snow days and during the summer.

FACTS operates the largest summer enrichment program in Flagstaff with daily participation averaging 300.

FACTS is supported by family fees, federal education grants, DES subsidies and the City of Flagstaff.

FACTS total budget this year is approximately $993,000.

FACTS provides full or partial scholarships (primarily through a subsidy of 315,000 from the City of Flagstaff) to more than 350 families with children in the program.

FACTS average daily per pupil cost for services is just over $9.00—more than $1.00 below the national average.

Are you familiar with the FACTS before- and after-school program?
If yes, how did you become aware of FACTS?
Approximately how many of your employees have elementary school-age children (ages 5-12) who might benefit from this program?
How important do you believe before- and after-school programs such as these are to our community?
We’d be pleased to see your specific comments or suggestions. Please enter your comments here:
